What would be the most likely cause(s) of the indicator being on? It doesn't go off after the engine is running, either. Thanks.

Most likely it is the *clock spring* ribbon cable that connects the components in the steering wheel hub to the steering column. If the horn and cruise control are not working, that is most likely the problem.
This was a common problem when the vans were re-designed for the 1996 model year. Not as common on earlier ones.
