fuel trim 28.9%???
in addition to the maf, check again for vacuum - tubes may be fine, but there may be a leaking diaphragm somewhere. and also look for clogged injectors. reset ecu when done and see what trim comes in at after.

But hey, read this:

And it goes lovingly on.
Admit it, it's sheer poetry!
HTML is really hard to read with a real newsreader.
Check your PCV valve and hose. Especially where it connects to the intake. Hose will crack at any bend. And, believe it or not the grommet at the valve cover will also leak. Both those codes together are usually a vacuum leak.
I bet a dollar there's a vacuum leak there. Clean the PVC, check the fuel pressure at the rail, and if those don't do it, start looking more aggressively for vacuum leaks. That might mean borrowing a smoke machine from someone. That 28.9% short-term fuel trim is not a good sign.

Will do.
