2006 Accord Hybrid Accelerates when Braking

2006 Accord Hybrid has intermittent problem of accelerating when braking at low speed. Honda is unable to recreate / validate the problem and there are no error codes to be found.

Does anyone have experience with this, or insight to the problem / solution?
