1990 Ford Taurus SHO TIMING BELT WALKING OFF TOP

1990 taurus sho timing belt walked off the top pulley (but only half way) s licing through the timing belt covers. Not sure why this happened. Occurred soon after new install of timing belt. Also installed new tensioner when t iming belt installed. Anyone seen this issue?
