Manual transmission on my '88 PU

Today I accidently shifted from 5th to reverse and ground the gear some. From that point on I lost the ability to shift into 5th or reverse, which of course are on the same plane. I took off the shift lever and cover and all three notches are lined up and nothing is noticably wrong, but no matter what, it won't go into 5th or reverse. It does push all the way over to the right though. Also, on the way home it seemed to lose syncronization(sp), that is, it shifts hard as if I'm trying to shift without the clutch. The clutch works normally though when you stop or engage it from a stop. The clutch is only a few months old. Do I need a rebuild or what?
