Harmonic Balancer go bad w/o separation?

Seems like HB failure is reported with inner and outer pulley relative shift, usually belt noise, throwing off the proper 1st order (?) damping of the HB to the crankshaft.

This implies that the crankshaft woodruff key only slots with the first pulley, but not to the second?

Can a HB fail - lose its counterbalancing properties - but pulleys not shift relative to each other?

Yes, the pulleys can come unsynced with each other, so the outside pulley has a wobble in it in relation to the inside one. The timing mark can still stay true.
