The problem turned out to be a cracked vacuum hose hiding under the upper intake manifold. It is hard to see. The hose is rubber, not one of those small plastic hoses. To help find a problem like this, spray carb cleaner around the hoses and under the upper intake plenum. Hope this helps someone else.
