Assuming that Code 25 is an indication of a lean condition, check for a crack in the intake hose between the air filter and throttle body, make sure all vacuum hoses are connected, and check your O2 sensor voltage and operation.

Assuming Ray was right (as he usually is) we can see that the 25 thrown could indicate a MAF or MAP problem, which is right behind the air cleaner. Check the connections and that the MAP is operating properly.

And the OP said it has a K&N air filter. Those are known to cause problems with Toyota MAF/MAP sensors, are they not?

I think perhaps the OP may have 'nudged' the MAP or a connector (?)

If going back to the stock filter doesn't fix the problem and everything else checks out OK, I'd check the O2 sensors, starting with the one after the cat.
