I was told by the vet that cats will act like this for a couple reasons. The most common is a dirty litter box like Doug said. Also the type of litter. The third is usually when they aren't feeling well. I have a cat, and she protests by digging the litter out of her box and on to the floor is I don't use "her" brand of cat litter. She has never done her duty on the floor, but she will empty the litter out of the box if it isn't the right brand.
