After 197,000 km my '91 MPV's heater output has dropped to about half the temperature that it usually puts out. This happened fairly quickly, maybe over a two week period. Any ideas where to start looking first?

Is the weather now cooler? Do you have a temperature gage? If so, what is it indicating?
My first thought is the thermostat may be stuck open, preventing the engine from reaching the normal operating temperature.
My second thought is the heater core may be gummed up.
My third thought is to check for any control valve that regulates flow to the heater core. These are often vacuum controlled.

Yes - all of a sudden!
Yes - engine is normal temperature.
