Will not shift into overdrive. Does anyone have any possible causes?

is it not shifting into overdrive, or is it giving you OD on top of second, which feels like third ?
how's reverse ?
It's not going into OD. Reverse kicks in good and works fine, as do 1, 2, and 3.
