-94 Voyager 4-speed transmission goes to neutral while driving

I have a -94 Chrysler Voyager with 4-speed transmission, and the car is fine except for the transmission. I can bear with the hard shifting between gears, but lately i have run into a worse problem - the transmission goes into neutral while driving! If I turn off ignition after the fault, I could drive for some 100 meters or so, then it happens again. This problem only occurs when the car has been driven for at least 30 minutes continously. If I let the car cool down, then it works like usual again. I have also experienced that the the transmission won´t change to third gear while driving - I always solve that by stopping and turning off the ignition, and start it up again. The first problem is a terrible one, the second is just bad. My questions are: Is the transmission going to break soon, is there some way to solve this problem?

It would be great help if someone could help me with this.

Could be tranny, could be control module, could just be fluid/filter need changing.

You do not mention the mileage (kilometers in your case) on the vehicle, but being a 94 I will assume pretty high. You also do not mention the last time you changed (or even checked) the transmission fluid and filter. In any case, if low fluid is not the cause then I would have a shop look at it. Even if it is the fluid, a shop would be a good idea to see where the leak is.
