Why is it headlights sometimes mist up? None of the ones I've seen is totally sealed, so why do some do it and not others?

Why is it headlights sometimes mist up? None of the ones I've seen is totally sealed, so why do some do it and not others?
they only mist when water has got in, tiny stone holes, failed lens gasket, careless pressure washing.
