I discovered that the inner disc pad on the right front wheel of my Previa is completely worn down, while the outer pad is still meaty and thick. I replaced them all about a year ago. Does this probably mean the piston is stuck (not retracting) and the caliper needs a rebuild, or could it be something less severe?

It means either the caliper cannot slide on its pins, or the outer pad is seized on the mount bracket.
Time for a brake service. Clean off all the rust and relube correctly.
turns out the anti-rattle springs were missing on the outer pad-perhaps they didnt slide as well as they should, there was a little corrosion on the contact points.
