Your local Toy dealer.
The short answer is yes, they make a 6 cyl manual shift, but you might have to suffer through getting the extended cab version. The PreRunner of a few years ago could be found with a 6 and standard cab, but it had an automatic -- all PreRunners had automatic transmissions. I do not know if the current run of PreRunners all come with automatic transmissions or not. The "standard" suspension models can be had with either an automatic or a manual, but the V6 might come only on the extended cab models. Personally, the extended cab is the only way to go, I'd not consider a standard cab at all. But, that's me.
Your local Toyota dealer can give you a definitive answer, and there is always toyota.com.
No, only a 4 cylinder in the regular cab pickup.

My 2007 Tacoma PreRunner has a 6 speed manual, so they are NOT all automatics.
