If I installed 20 inch rims on an f150 would it be necessary to use a spare tire that is the same size?

As was suggested, it is better to have the diameter of the spare same as your tires, but this is not a requirement. Look at the goofy doughnut spares that come with cars from the manufacturer. They do advise 45 mph speedlimits or such.
If you have a limited slip differential then you would need the same circumference tires on that axel to prevent damage to the differential.
