I've had relatives whose vehicle has passed with child safety seats fitted, and the fact noted that the rear belt(s) couldn't be tested for that reason.

There appears to be nothing in the "reason for refusal to test" that directly covers this; there really should be. The presenter should be required to remove them prior to the test.

My car is always returned with the passenger seat belts fastened. At least they have checked that they retract...

Nothing, I got advisories for a few things including a tyre that was feathering at one edge. At the time this would be back in 2003 - it may have changed now - they weren't allowed to remove child seats to test belts, and just marked them as "couldn't be tested"..

I had a car fail last year because of a chip on the plastic belt buckle cover.
