I used to work with a guy who'd reground the crank on the worlds largest oil tanker. I imagine that the engine was at least equal in size if not bigger than the one shown. He did say that you could stand on the piston and go up and down. Head off of course. ;-) They only shut the engine down up to the cylinder being worked on. The rest of it up, to the propeller, was still going.
I know people who have worked on engines where they can step into the cylinder via the valve :)
