if i install 31x10.5 tires on my 93 ext cab 4x4 do i need to change any gear ratios or ring and pinion or ne thing or is it ok stock?

The factory put 4.56 gears in the manual tranny trucks and 4.88s in the auto tranny trucks that came with the 31" factory tire option. You likely have
4.10/4.30 gears (manual/auto), but you can check to be sure:

Changing gears would help restore MPG and highway driving (like being able to use 5th gear) and would be a good time to install a locker or limited slip.
However, many folks run the tire with stock gears and have no problems.

31s will be fine with auto or manual, going up to 32s or 33s will start giving you issues not only with gearing but rubbing on inner guards etc.
33s will require a lift kit at bare minimum.
