I would replace the throw out bearing and keep using the same clutch. Provided you didn't tear the bearing up so bad that some pieces didn't go in and bust up the clutch.

The first question you have to ask is, "why did it fail?" If you don't figure that out you'll be replacing clutches every 7 months. The Centerforce is a high quality clutch, arguably the best according to some. But certainly high quality. So why did it fail? You can swap discs and pressure plates fairly easily.

But was the flywheel resurfaced? Is there any oil in there? When the flywheel was bolted back on, was thread sealer used on all the bolts? Maybe you have a rear main seal leak. But I suspect it was just a poorly adjusted clutch cable. That's usually the reason for an early failure. If that's the case, simply resurface the clutch and pressure plate, and put in a new disc. If there's any bluing or stress cracking of either, replace them.
