87 Maxima -- transmission problem: gear stuck in first... HELP!

Bought an '87 Maxima in 1998. In 99, it wouldn't shift out of first gear while driving, just would rev extremely high. Nissan put in a new transmission and it ran fine until April 2003.

Once again, the problem has reoccurred now. The tranny is stuck in first gear and will not shift out. Revs very high. Checked the transmission fluid, its fine.

Considering the knowledge in this forum, I come to you guys first, any help is GREATLY appreciated! What could be the problem? Has anyone else experienced anything like this with their 86-88 maxima?

NOTE: I take care of all fluids and maintainence regularily. The car had been running fine, until the incident.
