My wife was locked out while car was running. Its a GLE with keyless entry fob. This has never happened before. Luckily we had a spare door key. Any ideas why this happened or if it is an indication of some other problem? Thanks.

..yep it's a terrible thing to happen!.. Talked w/ the mechanic and he said another one had it happen too!.
I think I saw a service advisory about this.

I had a non-electronic Nissan key blank cut to match the ignition key. It's much, much cheaper than the electronic key; won't start the car but it opens the door. Put it in my (blond) wife's pocket book and haven't had a call since.
Don't know exactly how she does it, but she did it. Twice. I think if you open the driver's door then lock all doors, get out and close the door while holding the handle up all doors will stay locked.
Gotta love 'em ;-)
